ABSTRACT

Objective To evaluate the effectiveness of family management on improvement of management level for families with epilepsy children aged 0 to 6 years.Methods Families with epilepsy children aged 0 to 6 years hospitalized from April 1st to November 30th,2015 in our hospital were recruited and randomly divided into the control group and the intervention group.Within 24 hours after being diagnosed as epilepsy,general information questionnaire,epilepsy knowledge survey questionnaire and Family Management Scale(FaMM) were used for baseline survey.The control group received conventional treatment,nursing and health education.The intervention group received family management intervention for 6 months besides conventional treatment,nursing and health education.Epilepsy disease knowledge and family management were investigated at 3 months and 6 months after discharge.Results There was no difference between two groups at baseline (P>0.05).For different periods of the study,time factor and interaction existed in epilepsy disease knowledge and total score of FaMM(P<0.05).After intervention,there were significant differences in epilepsy disease knowledge and scores of FaMM between two groups(P<0.05).Conclusion Family management intervention can improve family management level of families with epilepsy children aged 0 to 6 years.The intervention contents are detailed and concrete,the intervention method is easy to perform,and the effects are significant,and the intervention is convenient for clinical application.

ABSTRACT

Objective To evaluate the effect of three methods on detecting cleaning efficacy of suction luminal de-vices in pediatric department,and find out a proper detection method for such devices.Methods Random number table method was adopted to take samples of suction lumen devices after cleaned in central sterile supply department in January-April 2016,visual observation with the aid of magnifying glass,jieli strip detection method,and ATP bi-oluminescence assay were adopted to detect cleaning efficacy of outer surface,screwed connection,and inner wall of lumen devices,the qualified rates of cleaning detected by three methods were compared.Results The qualified rates of suction lumen devices after cleaned detected by visual observation with the aid of magnifying glass,jieli strip de-tection method,and ATP bioluminescence assay were 99.00%,91.00%,and 71.50% respectively,qualified rates of cleaning detected by three methods were statistically significant(χ2 =71.545,P <0.001),pairwise comparison showed a significant difference (both P <0.0167);qualified rates of cleaning of the outer surface of lumen detected with three methods were all 100.00%;qualified rates of cleaning of screwed connection were 100.00%,98.00%, and 97.00% respectively(χ2 =5.695,P >0.05);qualified rates of cleaning of inner wall were 99.00%,92.00%, and 72.00% respectively(χ2 = 72.634,P < 0.01 ),pairwise comparison were all significantly different(all P <0.01).Conclusion The cleaning quality of the outer surface and screwed connection of pediatric suction lumen de-vices can be assessed by visual observation with the aid of magnifying glass,cleaning quality of the inner wall of lu-men can be detected by ATP bioluminescence method,when condition is limited,Jieli strip method can be used for routine detection,supplemented by ATP bioluminescence detection on a regular basis.

ABSTRACT

Resveratrol, as a natural polyphenolic compound, has a wide range of beneficial effects, which includes anti-tumor, cardiovascular protection, anti-oxidant and estrogen-like effects, and so on. Its various physiological properties are closely related to the therapeutic principle for prevention and treatment of high altitude hypoxia injury. Resveratrol may play an important role in relieving or curing high altitude diseases, especially high altitude polycythemia(HAPC). However, the literature about study and application of resveratrol in plateau medicine field is rarely reported up to now. In this review, we summarized the physiological effects of resveratrol, discussed the possible main principle of resveratrol for HAPC therapy, and looked forward to resveratrol's perspective or potential application in high altitude medicine.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of simple hypobaric hypoxia on parameters of hematology and blood rheology in order to establish a rat model of simulated high altitude polycythemia (HAPC) for the study of pathophysiologic mechanisms and medical prevention and treatment of HAPC.</p><p><b>METHODS</b>Forty-eight male Wistar rats were randomly divided into three normal control groups and three hypoxia model groups. Normal control group rats were bred in normoxia conditions, and hypoxia group rats were subjected to hypoxic exposure for 8 hours per day at simulated 5 500 m high altitude in a hypobaric chamber. After hypoxic exposure for 2, 4, 12 weeks, one group of normal control and hypoxia model rats were killed and blood was collected, respectively. Then parameters of erythrocyte and blood rheology were examined.</p><p><b>RESULTS</b>Mucous membrane of hypoxia model rats showed obviously cyanosis after 2 weeks hypoxic exposure. Hemoglobin concentration of hypoxia model rats were beyond 210 g/L after 2 weeks, 4 weeks and 12 weeks hypoxia exposure and significantly increased than that of normal control rats respectively. Besides, RBC counts, hematocrit, whole blood viscosity, erythrocyte aggregation index of hypoxia model rats were all notably higher than those of normal control rats respectively.</p><p><b>CONCLUSION</b>A rat model of high altitude polycythemia can be rapidly established by hypobaric hypoxia exposure at simulated 5 500 m high altitude for 8 hours daily.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the correlation between programmed death-ligand 1 (PD-L1) expression in primary lung cancer cells, tumor associated macrophages (TAM) and patients' clinicopathological characteristics.</p><p><b>METHODS</b>From 2008 to 2010, 208 non-small cell lung cancer patients who underwent surgery or CT-guided biopsy were recruited from Huadong Hospital, Fudan University. Immunohistochemistry staining was performed to evaluate the PD-L1 expression in both primary lung cancer cells and CD68 positive TAM. The relationship between PD-L1 expression and the clinical pathology was evaluated using χ(2) test. Spearman's rank correlations were used to determine the correlation between PD-L1 expression in tumor cells and macrophages.</p><p><b>RESULTS</b>Positive PD-L1 expression in primary cancer cells was found in 136 (65.3%) patients, which were negatively correlated with lymph node metastasis (P=0.009) and smoking history (P=0.036). Besides, TAM with PD-L1 expression (found in 116 patients) was positively associated with smoking history (P=0.034), well-differentiation (P=0.029) and negative lymph node metastasis (P=0.0096). A correlation between PD-L1 expression in primary tumor cells and non-small cell lung cancer associated macrophages was found (r=0.228, P=0.021).</p><p><b>CONCLUSION</b>PD-L1, secreted from TAM, might induce cancer cells apoptosis, and decrease lymph node metastasis.</p>

ABSTRACT

<p><b>BACKGROUND</b>Endometrial polyps (EPs) occur in approximately 34.9% of infertile women. Transvaginal sonography (TVS) is a routine, non-invasive component of fertility evaluation. Most ultrasonographic studies of EPs have focused on abnormal uterine bleeding; few have assessed EPs in infertile women. Furthermore, no studies have explored endometrial thickness and its correlation with EPs in infertile women. This study aimed to assess transvaginal sonographic assessment of endometrial thickness and its value in diagnosis and prediction of EPs in infertile women.</p><p><b>METHODS</b>A retrospective study on 314 infertile women was conducted from June to December 2010. After TVS, endometrial biopsies were obtained by hysteroscopy. Pathologically confirmed EPs were taken as the gold standard.</p><p><b>RESULTS</b>Based on recognized criteria, TVS had a sensitivity of 37.04%, specificity of 98.71%, positive predictive value of 90.91%, negative predictive value of 81.85%, and accuracy of 82.80% for diagnosing EPs. Mean endometrial thickness was significantly different in patients with and without EPs (P = 0.0001). In women in the mid and late-proliferative phase, the endometrial thickness was significantly greater in those with EPs than in those without them (P = 0.0001 and 0.024). Receiver operating characteristic analysis showed that endometrial thickness had a sensitivity of 85.2% and specificity of 38% in the diagnosis of EPs, the area under the curve being 0.64. In the mid-proliferative phase, sensitivity was up to 90.9%, the area under the curve being 0.70.</p><p><b>CONCLUSIONS</b>TVS is poor at detecting EPs in infertile women; however, transvaginal sonographic measurement of endometrial thickness is helpful. It is suggested that the diagnostic value of TVS for EPs in infertile women could be improved by adding the measurement of endometrial thickness to the variables that are routinely assessed.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To retrospectively analyze the effects of allogeneic hematopoietic stem cell transplantation (allo-HSCT) on childhood chronic myelogenous leukemia (CML).</p><p><b>METHOD</b>Of the 24 consecutive cases, 16 were boys and 8 were girls. The median age of patients was 12 (3 - 16) years old; 16 cases were in chronic phase (CP) of CML, 1 case in accelerated phase (AP) and 5 cases in blastic phase (BP). Allo-HSCT from HLA identical siblings were performed for 5 cases, HLA haplotype was performed for 14 cases and unrelated allo-HSCT for 5 cases. Twenty-four cases underwent allo-HSCT with conditioning regimen of BUCY. Prophylaxis of graft versus host disease (GVHD) included CsA + MTX plus MMF. The average follow-up was 36 months.</p><p><b>RESULT</b>All of patients were successfully engrafted. The 5-year overall survival (OS) of the 24 cases was 81%. Four patients died after allo-HSCT including 3 cases in BP from haploidentical donors and 1 case in CP from HLA identical sibling. The 5 cases who received unrelated allo-HSCT have been alive. Among the 10 cases who survived over 5 years, 3 had chronic GVHD.</p><p><b>CONCLUSION</b>Children with CML could be treated effectively with allo-HSCT. There were no significant differences among different donors. Transplantation to children with CML should be performed as early as possible. Preparative regimen adjustment before transplantation, the transplantation of associated comorbidities and effective prevention and treatment for CML patients after prolonged graft survival of high quality have important significance.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of rosiglitazone on serum high-sensitivity C-reactive protein (hs-CRP), interleukin-1beta (IL-1beta), IL-6, tumor necrosis factor-alpha (TNF-alpha) and insulin resistance in obese patients with newly diagnosed type 2 diabetes.</p><p><b>METHODS</b>This study involved 118 patients with newly diagnosed type 2 diabetes and obesity, who were randomly assigned into two groups for a 12-week treatment with rosiglitazone (4 mg/day, group A) or sulfonylureas (group B). Serum hs-CRP, IL-1beta, IL-6, TNF-alpha, fasting plasma glucose (FPG) and fasting insulin (FINS) were measured before and after the treatment. Insulin resistance index was calculated according to the HOMA Model.</p><p><b>RESULTS</b>In group A, rosiglitazone treatment resulted in significantly reduced serum hs-CRP, IL-1beta, IL-6, TNF-alpha, FPG and insulin resistance index (P<0.01). No difference in FPG was found between the two groups after the treatment (P>0.05), but serum hs-CRP, IL-1beta, IL-6, TNF-alpha and insulin resistance index were significantly lower in group A than in group B (P<0.05).</p><p><b>CONCLUSION</b>Rosiglitazone can decrease FPG, reduce the inflammation reaction and improve insulin resistance in obese patients with type 2 diabetes.</p>
